    Gift Aid Declaration Form

    Use this form to ensure your CAF Charity Account receives the Gift Aid available to UK taxpayers.

    Letter of Wishes

    Use this form to let us know how you would like the remaining funds in your CAF Charity Account to be distributed upon your death.

    Successor Nomination Form

    Use this form to nominate a successor and tell us which charities you'd like to send any remaining funds to when you pass away.

    Additional Signatory Form

    Use this form to add an Additional Signatory who can request distributions from your CAF Charity Acount and update details for an existing Additional Signatory.

    Signature Card

    For security purposes, we request a copy of your signature so we can verify Charity Vouchers and any other instructions we may receive from you in future.
